    Sports Information
     
    An updated physical, done within 1 year is required before the start of a sports season. 
                                  -Blank physical form (Physical Form) 
    Refer to the athletics page on the CCS website for start dates for each sports season. (Link to Athletics Page)
    Sports sign ups are done using FamilyID. Instructions for this can be found using the link listed above.

    Immunization Requirements:

    Immunization requirements for the 2022-2023 school year must be met by the end of the school day on Tuesday, October 4, 2022. Failure to meet these requirements will cause exclusion from school until proof of the necessary immunizations is turned in. 

    All 7th and 12th graders must be vaccinated with the Meningitis vaccination.

    All 6th graders must be vaccinated with:

    1)  Tdap (mandated after 11th birthday~ two week grace period.)

    2)  2nd varicella

    3)  4th polio (if 3rd given before age 4)
