Immunization Requirements for FALL 2017!

Proof of immunizations must be submitted to the appropriate health office by the end of the school day on Thursday, September, 28 2017. If the required documentation is not turned in by this date, your child will not be allowed to come to school until the necessary information is received.

 

Kindergarten immunization requirements

  • 3 to 5 doses of DTaP (diptheria, tetanus, pertussis) vaccine. Number of doses needed depends on ages DTaP immunizations were given. Check with your child’s doctor for number required.

  • 3 to 4 doses of IPV (polio) vaccine. A 4th dose is required if prior doses were given before age 4.

  • 2 doses of MMR (measles, mumps, rubella) vaccine

  • 3 doses of Hepatitis B.

  • 2 doses of Varicella (chicken pox) vaccine or proof of disease from your child’s doctor.

 

6th grade immunization requirements

  • 1 dose of Tdap (tetanus, diptheria, pertussis) vaccine booster.

  • 2nd dose of Varicella (chicken pox) vaccine or proof of disease. Most have already received this, but check with your child’s doctor to be sure.

  • 4th dose of Polio vaccine if 3rd dose was given before the age of 4.

7th and 12th grade immunization requirements

  • 1 dose of meningococcal conjugate vaccine (Menactra or Menevo)

  • Grade 12 students:  If the first dose of meningococcal vaccine was received at 16 years or older, the second dose is not required.
